经颅脑实质超声的黑质高回声诊断帕金森病的研究进展

摘要:
目前帕金森病(Parkinson disease, PD)的发病机制还未明确[1].有研究表明,黑质(substantia nigra, SN)是PD 病理生理学中重要的结构之一,甚至可以认为是唯一可信的与神经元丢失密切相关的脑结构[2].该中脑核团中神经元丢失和多巴胺含量缺乏通常导致运动障碍的发生[3].经颅脑实质超声(transcranial ultrasound, TCS)可以观察黑质结构,且具有无创、便捷、经济的优点,更易于在临床广泛推行开展.自1995 年BECKER 等[4]首次报告了PD 患者的SN 超声高回声性(SN+)以来,许多研究开始投入证实 TCS 对于PD 诊断及鉴别诊断的意义.目前欧洲神经科学联盟和我国最新的PD 诊断指南都认可了 TCS 的辅助诊断价值,但迄今为止,对 SN+形成的机制、统一的判定方法、PD 患者SN+的特点及诊断与鉴别诊断的作用等方面的认知仍没有十分明确.本文就现阶段关于TCS对PD的研究进展进行概述.
